1. Open the private link your care provider sent you (by WhatsApp, SMS or email) — it takes you straight to your records.
2. Or enter your access code below, then confirm your date of birth.

Either way you'll be asked for your date of birth. Don't have a link or code? Contact your care provider to request one.
